SELF CARE ASSESSMENTS
Below are links to tools and assessments that assess self-care as well as worksheets to develop a self-care routine. These worksheets and assessments are intended to be used as a tool to explore your self-care needs and are found to be best practices in the social work field. After completing the assessment and worksheet, and you feel like you need assistance with your self-care routine, please consider setting up an appointment with an EAP counselor to discuss your results. Please call 716-681-4300 or SUBMIT A REQUEST HERE. A counselor can help you understand the results and provide options for care.
